Intellectual disability (ID) in children is characterized by limitations in intellectual functioning and adaptive behavior, impacting everyday social and practical skills.
This condition arises from developmental challenges occurring before the age of 18 and is marked by difficulties in learning, reasoning, communication, and self-care.

Biological and Genetic Causes

A significant proportion of intellectual disabilities originate from genetic and biological factors. Conditions such as Down syndrome and fragile X syndrome represent genetic abnormalities that disrupt normal brain development. Fragile X syndrome, caused by a mutation in the FMR1 gene, impairs neural functions leading to cognitive deficits and behavioral issues. Additionally, errors in metabolism — including conditions like Lesch-Nyhan syndrome — and chromosomal anomalies contribute to varying severity of intellectual impairments.
Prenatal influences are equally critical; exposure to infections, malnutrition, or toxins during fetal development can interfere with brain growth. For example, inadequate oxygen supply during childbirth or preterm birth may result in neurological impairment. Notably, experts have identified that only about one-third of intellectual disability cases have a clear biological cause, highlighting the condition's complexity.

Psychological and Developmental Factors

Children with intellectual disability often exhibit delayed milestones such as late rolling, sitting, walking, and language acquisition. Psychological traits including difficulties in memory, problem-solving, and social comprehension are common. Lower IQ scores frequently assist in diagnosis, differentiated into ranges spanning mild to profound disability.
Moreover, children with ID may show struggles in connecting actions with consequences, affecting decision-making and learning from experience. Adaptive behavior—the capacity to perform social and practical tasks appropriate to one's age—is notably limited, necessitating varying levels of supervision and support. Severe cases might also involve communication challenges, requiring tailored educational approaches.

Environmental and Social Contributors

Beyond genetics and biology, environmental factors play a crucial role. Exposure to harmful substances like lead or extreme malnutrition during early childhood can cause or exacerbate intellectual delays. Additionally, neglect, or inadequate stimulation critically hamper cognitive and social growth.
Socioeconomic influences, such as limited access to healthcare and educational resources, further affect outcomes for children with ID. Early identification through developmental screenings improves chances for intervention, especially via community and educational support systems.

Classification and Spectrum of Intellectual Disability

Intellectual disability spans a broad spectrum, typically categorized as mild, moderate, severe, or profound, based largely on IQ levels and adaptive skills. Children with mild ID might face school learning difficulties but can often attain employment and social integration in adulthood. Conversely, those with profound ID display significant delays requiring constant care and assistance with daily activities.
